The Membership Application is for:

    • New Members or
    • Member who want to open a Separate Account

When submitting this form you will also need to submit:

    • Copy of Drivers License 
      (Primary & Joint if Appl.)
    • Initial Deposit of at Lease $25.00 
      (You can use the 'Electronic Transfer/Wire Form' for this.)

After Submitting this application, a representative will contact you to make sure all needed documents are received!

Loan Application

The Loan Application is for:

    • Existing Members Only 
    • All Non-Mortgage Loans

When submitting this form you will also need to submit:

    • Copy of Drivers License
      (For Primary and Co-Borrower if Appl.)
    • Income Verficiation
      (Usually, last year's 1040 is best. This is not needed for Share-Secured Loans)

After Submitting this application, a representative will contact you if you need additional documents. If you submitted all the necessary paper work, your application will be sent to our underwriting department for approval. In usually 2 business days, you will hear back with a response!

Transfer/Wire Form

This Transfer Form is used to:

    • Initial a One Time Transfer
      (Either an ACH Electronic Transfer or a Wire Transfer)
    • Set Up A Reoccurring Transfer
      (Such as a monthly payment from your account to a loan)
    • Update or Cancel a Reoccurring Transfer
      (Such as changing the date, amount, or account information of an existing transfer, or terminating it altogether)

Please Submit this Form IN IT'S ENTIRETYFailure to completely fill out the form may result in a delay, or a failure to process.

Add Service or Change to Account

This Account Change form is used t0:

    • Update Important Account Information
      (Such as a Name, Address, or Phone Number Change) or
    • Add an Account Service
      (Such as a Checking Account, a Debit Card, or a Savings Sub-Account)

When submitting this form you will also need to submit:

    • Copy of Drivers License
      (For Primary and Co-Borrower if Appl.)

After Submitting this application, a representative will contact you if you need additional documents. If you submitted all the necessary paper work, your account will be updated as requested. If there is any information that needs to be sent to you it will be forwarded to the address on file.

Change in Account Signers

This Change in Account Signer form is used primarily by:

  • LIBOR Chapter Accounts

In addition to this form, these documents are also needed:

  • Copy of Drivers License
    (For all New Signers on Account)
  • Letter Authorizing Change in Signers Issued from LIBOR
    (This letter should be received by LIRFCU staff directly from LIBOR staff)

After submitting the form, along with the needed documentation, you can expect the account will be properly updated, and all new account signers will received information required to access and utilize the account and account services.

Online Banking Access

This Online Banking Access form is used to:

  • Enroll Existing Members in Online Banking 

After submitting the form, you can expect to received an email detailing how you can access your account online. If you desire E-Statements, please request them through our Online Banking System. E-Statements will not be generated or saved to your Online Banking Account until you submit your request for them.

E-Statement Enrollment

This E-Statement Enrollment form is used to:

  • Gain Access to E-Statements on your Online Banking
    (You must be logged into your Online Banking Account to use this form.)

After submitting the form, your Online Banking Account will be set up to receive E-Statements on a regular basis. This E-Statements will be accessible through your Online Banking Login.
